Peninsula Home Hospice Caring for Residents in Aged Care

While funding for the Community Palliative Care in Aged Care Project concluded at the end of 2025, our commitment to supporting residents, staff and their families in Residential Aged Care Facilities (RACFs) continues.

A key achievement of the project was the development of strong partnerships with a significant proportion of the 55 RACFs across the PHH catchment. Through this engagement, we demonstrated both our commitment and expertise in supporting aged care staff in delivering high quality palliative care to residents with life-limiting illnesses, including those living with dementia.

Building on these established relationships, PHH is continuing to refine its model of care, which will incorporate both nursing and counselling support. In the interim, PHH has maintained a part-time Nurse Consultant role, with Tania providing ongoing specialist support.

Tania’s work with residents, families, and aged care teams is enhancing access to palliative care services and ensuring individuals in aged care receive compassionate, specialist support within their own environment.

The continuation of this role represents a meaningful investment in a vulnerable and often overlooked group within our community. It is a significant win for residents, their families, and the dedicated staff who care for them every day.
